To schedule COVID-19 vaccine appointment for kids from ages 6-months to 11-years old, please call the office.

AT THIS TIME COVID VACCINE APPOINTMENTS ARE OFFERED FOR DULLES PEDIATRICS PATIENTS ONLY! If your child is not a patient at Dulles Pediatrics, your online appointment booking will be CANCELLED without notification.

1) You must book an appointment online for the vaccination, click on "SCHEDULE COVID-19 VACCINE APPOINTMENT" below. Make a separate appointment for each patient who would like to receive a COVID-19 vaccine. If you do not see an available appointment please check back again.

2) Once you book the appointment look for a confirmation email from Dulles Pediatrics.

3) After you receive the confirmation email, complete the COVID-19 vaccination questionnaire on patient portal using separate login for each child. Click here on Patient Portal to complete the questionnaire. Once you login to patient's account, select "Fill Questionnaire" under APPOINTMENTS, enter your responses and click on "Submit".

Note: the questionnaire must be completed for each dose of COVID-19 vaccine, and before you arrive to the appointment. Vaccine will NOT be administered until the questionnaire is done.

4) Before you come to the appointment, please read the consent forms here COVID-19 Vaccine Consent Forms . You will be signing these forms in the office for each COVID-19 vaccine before it is administered.

NOTE: If vaccines are not available due to a delay in shipment from the health department, we will contact you to reschedule the appointment.

After the patient receives the COVID-19 vaccine use your smartphone to inform CDC about any side effects. You can sign-up with your smartphone's browser at vsafe.cdc.gov
